The online application must be completed at one time and cannot be saved in progress to be completed at a later time.

Some of the information requested will help us determine whether the student meets the criteria for entry into the program, and some help us know best how to assist your student if they become a participant. You are responsible for making sure that the completed forms are returned to the Math and Science Center office.

The following parts of the application must be provided to the Math and Science Center office in order for your application to be considered:

• Application

• Financial Eligibility Information
  A copy of a tax form or other verification of monthly income must be attached!

• Counselor or Teacher Recommendation

• Transcript

Applications must be completed before a student can be considered for admission to the program.
